Please bring the printed forms with you to your appointment.

This lets us know the history and current state of your health, any questions or concerns you may have regarding wellness, and any other details we should be aware of. Be sure to let us know!

Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options.

Chiropractic History and Examination

In order to determine what your actual problem is, the doctor will ask you various questions related to your condition. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent. Technology called the surface electromyograph may be used to determine areas of stress on your nervous system.

Chiropractic X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious chiropractic x-rayconditions or assist us in developing the most effective treatment for you.

Same Day Treatment

Once finished with the consultation and examination, most patients will be treated at that time. This may include chiropractic adjustments and soft tissue therapy.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches or other chiropractic techniques.

Schedule Your Next Appointment

In order to achieve the benefits of chiropractic care, you will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.
